چکیده

Introduction Precise assessment of left ventricular (LV) contractility during veno-arterial extracorporeal membrane oxygenation (VA-ECMO) is crucial. However, changes in loading conditions may mask changes in LV function when assessed with load-dependent indices like LV ejection fraction (LVEF). Indeed, these indices do not reflect intrinsic LV contractility but are the result of interaction between preload, contractile function and afterload [1].

Left ventricle unloading during veno-arterial extracorporeal membrane oxygenation

Extracorporeal membrane oxygenation in veno-arterial configuration (VA-ECMO) is a method widely used to support circulation during the most severe conditions of heart failure or refractory cardiac arrest. It frequently bridges this otherwise fatal status to recovery, therapeutic intervention or other circulatory support. However, the use of VA-ECMO in critical situations of severely impaired he...

Successful Management of a Patient with Refractory Ventricular Fibrillation (VF) due to Acute Myocardial Infarction (AMI) and Lung Injury by Transition from Percutaneous Cardiopulmonary Support (PCPS) to Veno-Venous Extracorporeal Membrane Oxygenation (ECMO).

A 69-year-old man was admitted to our hospital with cardiopulmonary arrest. Percutaneous cardio-pulmonary support (PCPS) using the right femoral artery and vein was initiated, because ventricular fibrillation continued. Although we succeeded in defibrillation after percutaneous coronary intervention (PCI), a chest radiograph indicated a pneumothorax in the right lung and a pulmonic contusion in...

Successful ECMO-cardiopulmonary resuscitation with the associated post-arrest cardiac dysfunction as demonstrated by MRI

BACKGROUND Veno-arterial extracorporeal membrane oxygenation (ECMO-CPR) is a life-saving rescue for selected patients when standard cardiopulmonary resuscitation fails. The use is increasing although the treatment modality is not fully established. Resuscitated patients typically develop a detrimental early post-arrest cardiac dysfunction that also deserves main emphasis. The present study inve...

Lactate clearance for initiating and weaning off extracorporeal membrane oxygenation in a child with regressed left ventricle after arterial switch operation

We hereby report a child with transposition of great arteries and regressed ventricle who underwent arterial switch operation (ASO) with the aid of cardiopulmonary bypass and "integrated" extracorporeal membrane oxygenation (ECMO) circuit. The significance of lactate clearance as a guide to initiate and terminate veno-arterial ECMO in a post ASO child with regressed left ventricle is discussed.

Disrupting differential hypoxia in peripheral veno-arterial extracorporeal membrane oxygenation

Patients receiving circulatory support with peripheral veno-arterial extracorporeal membrane oxygenation (VA-ECMO) are at risk of developing differential hypoxia. This phenomenon occurs in patients with concomitant respiratory failure. Poorly oxygenated blood, ejected into the ascending aorta from the left ventricle, competes with retrograde flow from the ECMO circuit, potentially causing myoca...
